• Before you start exercising with me, you will need to complete a TFF ParQ form which includes a health questionnaire. Please download and complete this form before bringing it along to your first session.
• By completing this form or contacting me about your interest, you are consenting to me using the information provided to recommend exercises and activity suited to your health and fitness.
• Please note that by law I need to store your PARQ form regardless of how long you exercise with me.
• If for any reason I am concerned I will ask you to consult your GP before continuing with exercise.
